Anti-IgE:IgE complexes may transform CAR T-cell treatment for refractory autoimmune diseases. By leveraging AI-assisted bioengineering strategies—including multi-specific “Triton” nanobodies, logic-gated encapsulation, structural conformance tuning, and co-delivery with tolerogenic peptides—this approach may enable localized immune modulation, reduce systemic toxicity, and potentially achieve a permanent autoimmune reset. Integrating AI design tools accelerates both mechanistic understanding and the translational potential of anti-IgE:IgE complexes, offering a new paradigm for safe, precise immune regulation in refractory autoimmune disease.
